Dr.
Judith L. Kirstein, Family Practice
Dr. Kirstein grew up in Los Angeles, California. Her father was an auto
electrical mechanic and her mother a nurse. Dr. Kirstein knew at a very
young age that she wanted to be a doctor. However, the opportunities for
women in medical schools were few at that time. Thus she elected to earn
her Bachelors and Masters degrees in nursing before changing her focus to
her first love, medicine. She received her Medical Degree from UCLA, and
then came to Salt Lake to the highly-acclaimed Family Practice Residency
program at the University of Utah. Since July 1981, she has been practicing
in West Jordan, Utah. Dr. Kirstein founded Jordan Valley Family Health in 1984.
She is a member of Alpha Omega Alpha Medical Honor Society, a fellow of the
American Academy of Family Practice, and a member of the Utah Medical Association,
the Salt Lake County Medical Society and the AMA. Dr. Kirstein is board certified
in the specialty of family practice. She is also associated with the clinical
faculty of the University Of Utah College Of Medicine. She and her husband,
Bob, live in South Jordan.
"I am glad to have selected Family Practice as my specialty, as it allows me
to know and understand all members of the family. Thus, I am able to help the
entire family with their medical as well as emotional needs. At times I feel
more like a family friend. My philosophy of patient care focuses on involving
each patient in the decision-making process. I tell my patients their options
and help them choose the appropriate one for them. My hobbies include cents-off
couponing and refunding, travel, camping, museums, and swimming. I have a
miniature schnauzer and a husband. However, I don’t know which is harder to train."
Dr. Kirstein has completed her specialized training in medical acupuncture through
UCLA's course for practicing physicians. She will be offering this age old and
effective treatment along with traditional western medicine. As the old cliché
"Were East meets West" is referring to cultures or boarders, here we are talking
more of a return to the concept of total physical, emotional and spiritual well
being rather than the isolated disease specific auto mechanic approach to your

At
this time Dr. Kirstein is not accepting new patients in her traditional
practice. However, she is accepting new patient for acupuncture treatments.

Dr.
Joni Hougaard, Family
Practice with Obstetrics
Dr. Hougaard was born in Cottonwood Hospital,
but grew up in Ely, Nevada. She was the Valedictorian of her high school
class and went on to Brigham Young University for a degree in zoology. During
1987-89, she served an LDS mission to Oslo, Norway, then attended medical
school in Reno, Nevada, followed by a three year residency at FHP. She began
working part-time at Jordan Valley Family Health in October 1996, and full-time
in July 1997.
Dr. Hougaards interests include golf, piano, travel and watching her
more than 200 videos. Her husband, Todd, is a news photographer for Fox
News.
"I chose to do Family Practice as I enjoy caring for a variety of patient
problems and conditions. I feel its important to deal with an individuals
problem in the context of his or her family."

Dr. Dennis Hamp, Family Practice
Dr.
Hamp joined our practice in August and is accepting new patients. Dr.
Hamp grew up in the Salt Lake area. He has lived in West Jordan, Taylorsville
and West Valley City. He recently completed his family practice residency
training at Southern Illinois University as its Chief Resident. He and
his wife, Elma, have four children, the youngest born this last January.

Dr.
Dave Lorah, Family Practice
Dr.
Lorah is originally from Glenwood Springs, Colorado. He earned a
bachelors degree in environmental biology from the University of Colorado
at Boulder, and his MD at the University of Colorado Health Science Center
in Denver. He completed his medical training at the St. Mark's Family
Practice residence program here in Salt Lake City, where he served as
chief resident. He joined JVFH in August 2001 and is currently accepting
new patients.
"I
enjoy the scope and variety of family practice as a specialty. The
opportunity to treat many generations in the same family - from pediatrics
to geriatrics is always a joy." Outside of medicine, his hobbies
include playing the classical guitar, snowboarding, camping and hiking
Utah's beautiful outdoors and, especially, spending time with his family.

Dr. Caryn Kendra-Slack, Family Practice
Dr. Slack was born in Massachusetts and raised in Fairfield, CT. She was always interested
in healthcare as a child, which may have been due to her Italian grandfather being a general
practitioner in the Bronx. He practiced from the basement of the family apartment, and did
many home visits to Little Italy in lower Manhattan at the request of his immigrant parents.
Dr. Slack received her bachelor of arts degree in history with a minor in French from
Georgetown University in Washington DC., and her medical degree from the University of
Minnesota in Minneapolis. Prior to residency training, she did a year internship in
general surgery at the University of California, Davis. Dr. Slack then moved to Connecticut
and entered the family practice residency training program at the University of Connecticut,
Hartford. During residency, she was appointed Chief Resident and completed a national ski
patrol course. She joined our team at JVFH in December of 2005.
In her free time Dr. Slack enjoys traveling, skiing, studying Polish, French and Spanish,
On top of that she is raising 2 dogs, one parrot and is expecting a baby girl at the end of
February 2006. She is also a stepmom to her husband Steve’s two children every weekend.
Dr. Slack enjoys providing a full range of family care, and has a special interest in natural alternatives for common medical problems.

Nurse Practitioners
and Physician Assistants
Michelle
King, Family Nurse Practitioner
Michelle grew up in Salt Lake, attending
Brigham Young University and then the University of Utah, where she earned
her Bachelors and Masters of Science degrees in nursing. After completing
a residency in Salt Lake in 1980, she went into practice in the West Valley
area. Three years later, she came to West Jordan and assumed the busy
practice of Dr. Gordon Sollereder, a family physician who wished to return
to his home in Canada. She and Dr. Kirstein combined practices in 1984.
Shes a member of the American and Utah Nursing Associations and
is also an Auxiliary Clinical Instructor for the University of Utahs
College of Nursing.
In her spare time, Michelle enjoys racquetball, snow and water skiing,
tennis, scuba diving and handicrafts. Michelle and her husband Lyman,
have two children: Cameron and Sierra.
"I am not the television news anchor-person, though some day we should
switch places for the day. Ha Ha! I really enjoy teaching preventative
health and participating in the effort to improve your health and lifes
enjoyments."

Amy LaDue, Family Nurse Practitioner
Amy grew up in Phoenix, Arizona.
She attended Westminster College for her undergraduate degree in Nursing,
graduating in 1977. She has practiced nursing in a variety of roles;
pediatrics in the Semi-ICU at Primary Children's Hospital, labor & delivery
at Holy Cross Hospital (now Salt Lake Regional) and St. Marks Hospital,
and home care. Prior to college She became a respiratory therapist
and practiced in Arizona and at Primary Children's while attending college.
Completed her masters of nursing and family nurse practitioner in 1997 at
the University of Utah. She did her residency with Michelle at Jordan
Valley Family Health. "I enjoyed the practice so much and was
thrilled with the opportunity to stay on."
She
married John in 1976 and they have 2 children, Lisa and Scott, both attending
college now. "We have had an empty nest for a year. My family
is active in skiing, hiking, biking, river rafting - anything outside
- I have a hyper family, which I love trying to keep up with. John and
I enjoy traveling. I also enjoy reading, quilting, cooking, and
taking time to be with friends. I traveled to Vietnam in the spring
of 2000 and did my first medical mission trip primarily serving orphans
- they taught me a lot by their eating and lifestyle habits - little portions
using chopsticks only and ride your bicycle or walk."
"The
most fulfilling aspect of my work as an FNP is educating, helping patient
to understand what is happening to them and how they can help themselves.
It has been an honor and a joy to see whole families and individuals over
time and to share your lives."

Joanne
Haeffele, Family Nurse Practitioner
Joanne
and CJ job share and primarily in the afternoon and evenings. Joanne is
a clinical instructor for the University of Utah, College of Nursing and
practices part-time at our clinic. Her scope of practice include
extensive family practice care, including obstetrics and gynecology with
management of both acute and chronic problems. However, at our clinic,
Joanne does not deliver babies. Joanne obtained her diploma in nursing
at St. Annes Hospital School of Nursing, Chicago, IL in 1976, a bachelor
degree in nursing at Barat College, Lake Forest, Il and her masters degree
as a primary care family nurse practitioner at the University of Illinois
at Chicago, Chicago, IL. She has several publications to her credit, including
books and journals. We are fortunate to have Joanne as a member
of our medical staff team.

Todd
Schauers, Physician Assistant
Todd
has recently completed his physician assistant training at Emory University
in Atlanta, GA. Prior to entering the PA program, Todd worked at
our clinic for two years as a medical assistant in preparation for his
advanced training in clinical care.

Richael
Egbert, MBA, Administrator
Richael
obtained his B.S. degree in Management and Finance at Brigham Young University,
Provo, Utah. He went on to complete an MBA degree from the
University of Phoenix, Phoenix, Arizona in 1989. During his studies he
worked in various banking and financial institutions gaining experience
in finance. From college he became the administrator of multiple clinics
for the FHP integrated HMO that was operating in Utah at that time.
When FHP dismantled and sold the clinics, Richael migrated with several
former FHP management individuals to Boise, Idaho and help establish a
network of physician offices and urgent care clinics. He has also been
the Chief Operating Officer and Administrator for several professional
organization. We are very fortunate to have such a highly qualified individual
take the helm of our organization and further improve the services you
will receive.

Allied
Service Providers
Allied service providers are individuals or companies who have contracted
to provide specialized patient care or support services. This is another
extension of our commitment to quality healthcare for the entire family.
Brenda Reiss-Brennan, MS, APRNCS
Primary Care Family Therapy Clinics, Inc.
Brenda grew up in Long Island,
New York and attended St. Dominics High school. She attended Niagara University,
New York, and then came west to Utah for graduate school at the University
of Utah. She earned the psychiatric nursing clinical practice award in
1996, and is a member of the Society for Education and Research in Psychiatric
Mental Health Nursing. Brenda is certified as a clinical specialist in
child and adolescent psychiatric mental health nursing. She and her husband,
Gerard, have five boys. They enjoy skiing, swimming, biking and music.
